Excel-type form

    This may be complicated so pardon the length.

    Let's say our organization has 150 different lines of business.  Whenever a user puts in a training request (parent table) we want to know whether to include any/all of the 150 different lines of business in the training (child table).  There are also internal and/or external locations for each line of business so we want to ask if we need to train the internal/external agents. .  And let's finally say there are two other questions asked for each line of business. 

    If you were to visualize this in excel, you'd have one column for the lines of business and then 4 additional columns to capture the yes/no values for the various questions asked.

    Finally, let's assume that I had another table which captures all the different LOBS (as records).

    1. Is there a way to name a field based on the value of a record in another table (so if the Line of Business changes names, could it change the field name)?
    2. What's the best approach to mirror a QB form like excel so that the respondents see all the different LOBs and can answer the 4 questions for each LOB?

    Let's start there for the framework.  Additional questions may follow as I want an element (training duration) from the parent record to show for each LOB if the answer to any question is "yes".

    I would set this up with a many to many relationship.

    One LOB has many LOB Training Assignments
    One Training Topic has many LOB Training Assignments

    The LOB Training Assignment would have the 4 questions

    I would I would contract with me :) for an hour or so of consulting time to implement a Quick Select URL formula button to allow the user to select a focus Training Topic and then quickly select all the LOBs for which a join record - (LOB Training Assignment) needs to be created. 

    Then I would have a report of LOB Training Assignments needing some or all of their questions answered.  That update might be easily done Grid Edit mode.
