Hi, 

Is it still possible to create an exact form?  Is the template supported.

I kept an old template on my desktop, but after clicking on Add-On, I can't seem to sign on to Quickbase, just get a mismatch error?

Exact forms are still supported and can be downloaded from this article (you may need to get the most recent version):

If you're looking for the 64 bit version, you can submit a support case requesting it.

Turns out I was using an out of date template.

Have since got the updated template.

But now I find the form only accesses and displays the field values if I turn the app tokens off.....

Correct, it is documented that exact Forums doesn't play well with app tokens.

If you must have application tokens, then you need to switch to something like Juiced EF+.
