Can someone help me set up a URL formula button to to push a record to another table?

I am trying to create  a button that will send a record from one table to another. Ideally it would auto fill part of the form in the other table and then I can fill in any additional information. I am using a URL formula field to create the button but can't seem to get the code right.

I know I have to probably do two formula's. This is what I have and it is solely constructed off of what I found off google. I am not the best coder and can't find a base layout that outlines what needs to go where.

I am trying to send an incident in the incidents table to  become an intake record in the demand intake table.

I put an app in the Exchange called URL formuals for Dummies, that explains in more detail, but here is an example:

urlroot() & "db/" & [_DBID_of the table name goes here in square brackets that you get from Advanced properties] & "?act=API_GenAddRecordForm"

& "&_fid_10=" & URLEncode([some field that might have spaces]

& "&_fid_11=" & totext([some numeric field]

& "&_fid_12=" & totext([some date field]




so that will pop up an Add Record form and pre-populate field ID#s 10, 11 and 12.  You get the field id numbers off the field list or else the usage tab on each field.
