Customer Field Drop-down

How do you set-up Customer Field where you have to choose between Individual and Company.   When Individual is chosen, two separate fields for Last Name and First Name will appear.  When Company is chosen, a single field will appear where you can put the company name.  Need help on this, please!  Thanks!

I suggest that you show all three fields on the form.  The user will input into which ever fields they have info for.  Then in View mode on the form and in reports use the formula field called
[Company /Individual]

List("\n", [Company], List(" ", [First Name], [Last Name]))

That should show the company name, and then on a new line the Name.  If either are blank then it will be suppressed.
