The purpose of the application is to display images and have users choose an image.  There are a lot of such images and I would like to give the person conducting the questionnaire some control of the images and titles.

There are 60 questions/images.

I've set up a separate table in the same application for the pollster to upload the photos and title them.

In the table the voters will see and make their selections, there is a field for the pollster to indicate the Record ID of the photo an I am using that in a Formula - Text field to display the image.

"< i m g width=\"800\" src=\"https://company.quickbase.com/up/tableid/a/r"&[Field 1 Image Number]&"/e9/v0/>"

What my question is here is how do I use a Formula - Text field to display FID8 (text) of the same RID?

Try this

"< i m g width=\"800\" src=\"https://company.quickbase.com/up/tableid/a/r"&[Field 1 Image Number]&"/e9/v0/>" & " Image # " & ToText([Field 1 Number]

Thanks Coach.  I did not explain well enough.  It is a two table application.  There are sixty of these where there is a title, an image, an image number and a multiple choice on each one.  In a separate table is the image and title.  The Pollster types in the image number in the main table which is the Record ID of the other table where the jpg file is uploaded.

My example is the Formula Text field displaying the image uploaded in the other table where the record ID equals the image number typed in to the local record.  What I am looking for is how to get the title field from the same record in the other table that the image is uploaded to displaying in the Formula Text field in the main table.

My other option is to have the Pollster just type it in and have the field read only to the voter.
I'm  not really understanding your setup, but if you invite me to the app I would be happy to take a look

An invitation is on its way.  This is still under construction.  What I am looking for is to have Field 1 Title in the Defect Image Voting table to have the value of the Defect Name field in the Defect Images table where the value of the Defect Image 1 number field matches the Record ID of the Defect Images Table.

The purpose is to have a person in the "Pollster" Role upload images to and key in Defect Names to the Defect Images table then merely match the images and names by inputting the Record ID into the pertinent Image Number fields in Defect Image Voting
I made a relationship so instead of just keying the image #, they are choosing related image.  So same data entry, just choosing from a drop down.  Or if you did just want them to key a number, then they could key a number and then have related image be a formula field equal to that data entry field.  The Image name pulls in on a lookup.

Is that what you wanted to achieve?

Thanks for the help.  Very helpful.  I was hoping to avoid building 60 relationships but it looks like that is what I need to do.
Well, whenever I find myself building 60 copies of five fields and 60 relationships, the answer is that the app should be considered to be rebuilt with a Patent and then X number of child records.

So build a model record with your questions and I forget how many there were, but then use the copy master detail button to create the children because all of your children are in fact identical and have the user fill out the survey by working the way down through the child records.
