Help! I need to write a multiple condition statement where my Formula checkbox is checked

To explain,I'm trying to capture the Calendar Year 2016 entries with a formula checkbox field (CY'16) for each record

These 7 categories come from the 'ITPG Project status' drop-down field:

1. Completing Request for Due Diligence
2. Approved to Proceed with Due Diligence
3. Project Approved
4. Fast Track Approval
5. Did Not Meet Project Criteria - Operational Approval
6. Request Denied
7. Request Withdrawn

How do I write it all in 1 formula using he OR statement so I don't have to create multiple CY16 checkboxes given each record is only captured in one category at once?

1. Completing Request for Due Diligence  - date comes from the 'initial inquiry date' field

2. Approved to Proceed with Due Diligence - date comes from the 'Initial project proposal submission date' field

3. Project Approved -  date comes from the 'ITPG final project approval date' field

4. Fast Track Approval -  date comes from the 'ITPG fast track approval date' field

5. Did Not Meet Project Criteria - Operational Approval - date comes from the  'ITPG Operational approval date' field

6. Request Denied - from the 'Request Denial date' field

7. Request Withdrawn - from the 'request withdrawal date' field

This is what I started with...

Case( [ITPG Project Status] = "1. Completing Request for Due Diligence" and [Initial Inquiry Date] >= ToDate("01-01-2016") and [Initial Inquiry Date] <= ToDate("12-31-2016")

or [ITPG Project Status] = "2. Approved to Proceed with Due Diligence" and [ITPG Proposal Approval Date] >= ToDate("01-01-2016") and [ITPG Proposal Approval Date] <= ToDate("12-31-2016"))
