How do I make a web form and is it possible to load it to site? Is there a spot for html to load it to a site as well?

Hope you are not new to Web Development.  You can make your own web form easy enough.  Simply add the fields that you want in your html form and bind your submit to Quickbase's API_AddRecord.

One challenge here is with authenticating in Quickbase.  For that, you may need to ask for the user's username and password.  If your intent is to offload users from your Quickbase account (cost savings?), then you will need to authenticate with some service account.  I strongly recommend obscuring those credentials by first passing this form data to a custom web service and having your server authenticate on its end.  Anything you put on the page is readable, so you should not include credentials there.

If saving on user licenses is what you are after, there may be a better way.  Rather than trying to custom code all of these web forms, you can instead deny users who have not used Quickbase in some time.  I wrote a script to do this, and a second script to automatically give access back when they sign in.  It works very well in streamlining this process.
