How do we create a parent/child report that shows all the records from the child table, organized by parent?

Just do the report off the child table and bring down any lookup fields you need from the Parent. You can make a text formula field if you like in order to have an informative heading break for your "Group By" in a table report type.

Well, I'm not sure what you data fields are from the parent, but let's say that the parent was a Project and the children were tasks.
