How to format the display of a text(formula) field?

Hi I would like to know how to display the value of a Text(formula) field to look like in the attached file. I have manage to display the values of other field inside the text(formula) field using the formula below. but its just a one continuous line on all of the values that i want to display, i would like to know what to place at the end of the line to say the this next value should be in another line. thanks in advance.
