I have a table where users plan tasks by selecting the planned and completion dates. But my exact form does not show all of the dates entered by the users.

if those fields for the dates were added after the exact form was coded, it may cause the items to not appear properly.  

Another cause could be the formatting of the document template itself.

Also, make sure they save the record, prior to 'generating' the form.

Just some ideas.
