William - This looks to be an issue that is specific to your application. For access issues, you need to reach out to your manager or, if you know who they are, the person responsible for maintaining that application.

Douglas - This forum is comprised of QuickBase users across the entire QuickBase customer base, not just Intuit. For access to QuickBase applications, you typically should speak with your direct manager. If they are unable to provide that access, you can also submit a ticket to the QuickBase Support Team who may be able to tell you who the manager of the application is.
