Limited One Record Per Person

I have a survey form that will be accessed via link. How do I make sure one person is only able to submit one survey?

If they are entering their email, you can make that a 'unique' field, thus no duplicate emails can be used.

(Or any other unique identifier)

The higher plan level of Quick Base, "Platform" lets you limit access to the Realm by IP address.

Also would the ability to limit access by IP live under the Roles section?

No it is not by Role.  It is by Application under a setting 
"Only users logging in from "approved" IP addresses may access this application (what does this mean?)"

But the actual list of IP addresses is specified globally for the Realm.

You will need to locate the person in your organization who has access to the Account Properties and there is a Summary tab there which gives the plan level.  Or I guess Quick Base Support can tell you.

Even if you find out your plan, this wont help with your question/issue.  
The IP address can get reused so many times, and even the same user might have 3 different IP address. How will the app know if the IP address has already entered a record? etc.

basically don't bother going this route.

Are the people entering the Survey QB Users?  or are you using 'everyone on the internet'?

They will be everyone in the internet

Then you will need to collect some unique identifier.

Email is usually the best option.

Or you can have a combination of fields be unique.
