Need help with Formula for Date Field

I have a date field called "Return Date" and 4 checkboxes listed as 30 days,60 days,90 days and 120 days.

Based on what checkbox you click (for example a person clicks 30 days and the return date formula field changes to 30 days out from current date and so on for each check box) Is this formula possible?

if you click check box "30 days" the formula field "return date" changes to 30 days away from today.

Is there any date on the record on which these checkbox fields can be based other than today.  For example can we anchor on the [Date Created] field.

I have a date field called "requested date" which defaults to todays date, would that work?
I suggest cgnaging the Return Days to a multiple choice field, if it's not to late for that.  You can set that to be a radio button if you like. That way a user only gets to choose one choice

Not tested but try this as a formula-date field type for Return Date

[Date Requested]
+
Case([Return Days],
"30 Days", Days(30),
"60 Days", Days(60),
"90 Days", Days(90),
"120 Days", Days(120))

If you are really adverse to getting rid of those 4 checkbox fields then,

+IF(
[30 Days], Days(30),
[60 Days], Days(60),
[90 Days], Days(90),
[120 Days], Days(120))
