require a field when another field changes

I have a field "Issued By" and another Field "Change Reason". I want to set it up so that any time someone changes "issued by" they must also enter something in the "change reason" field.

I've tried using rules "When the record is saved" and "issued as has changed" but there isn't an option that seems to fit. I'm fine with aborting the save, but I don't see a way to do this.

When [Issued By] is not equal to (leave blank)

Require [Change Reason]

what if i want to uncheck the [Date Confirmed] check box whenever the [Install Date] changes?

Example: [Install Date] = January 15th. [Date Confirmed] = checked/true. then i get a phone call that says we need to change the [Install Date] to February 19th. when i change the [Install Date] i need the [Date Confirmed] check box to become unchecked/false until i can later confirm the new date with the install crew, at which time i will manually check the box again so we all know that the new date has been confirmed.

thanks David, i sure will. it seemed like a pretty similar question so i didn't want to clog the board with a question that is similar to one someone else already asked.
