Is it possible to select a different form based on a field selection

Try something like this using a formula URL field::

Case([field],
  "foo", "https://SUBDOMAIN.quickbase.com/db/DBID?a=ACTION&dfid=5",
  "bar", "https://SUBDOMAIN.quickbase.com/db/DBID?a=ACTION&dfid=6",
  "baz", "https://SUBDOMAIN.quickbase.com/db/DBID?a=ACTION&dfid=7"
)

You will have to fill in SUBDOMAIN, DBID and ACTION placeholders.

This was an excellent answer, I am trying to use this same type of formula for one of my applications. The difference is I have 2 forms on one table, one is for a Week 2 checklist, one is for a Week 4 checklist. After someone has created a checklist, I need to have a button that will allow them to view or edit the checklist on the correct form. Here is my current formula:

Case([Checklist type formula],
  "Week -2 Checklist", "https://SUBDOMAIN.quickbase.com/db/DBID?a=nwr&dfid=2",
  "Week -4 Checklist", "https://SUBDOMAIN.quickbase.com/db/DBID?a=nwr&dfid=11"
)

Which is enabling a blank form to be opened in the correct form type. Is there a way to encode the record ID of the record into this formula so that it can edit that record in the correct form?
