I would like to use a concatenated field as my record title on the form.  However, the title shows up as the function/formula...not the concatenated field.  Please advise.

That sounds impossible.  Can you post your "formula" and tell us if this is a text formula field type?

Concatenated Field:
var text MyConcatenatedFields =
List(" | ", [MEETING TYPE],ToText([MEETING DATE]));
If(
[MEETING TYPE]="STARTS MEETING", "<b><font color=#2554C7>"& $MyConcatenatedFields & "</font color></b>",
$MyConcatenatedFields)

This is what shows as the "record title":
<b><font color=#2554C7>STARTS MEETING | 10-03-2016</font color></b>

A record title may not contain HTML.  You will need to have a version of the formula like

So one version to use elsewhere in the app with  color and another field for the Record Title without color.

List(" | ", [MEETING TYPE],ToText([MEETING DATE]));
