I recently recreated this demo to explain to a client how the Image Onload Technique (IOL) worked.

Simplest IOL Example

Basically when you enter text into the [Name] field it will be automatically converted to upper case when the field is blurred.

Here is the code the page is using:
(function(){
  var querystring=document.location.search;
  if (/a=nwr/i.test(querystring) || /a=er/i.test(querystring)) {
    $("#_fid_6").on("change", function() {
      $("#_fid_6").val($("#_fid_6").val().toUpperCase());
    });
  }
})();

It just so happens that this example of converting a field value to upper case was the original example I used when developing the IOL technique years ago.
