when I added dynamic form rules all of my elements disappeared. Where did they go?

I can still find all of the information in the "Fields" tab, but when I click on "Client" to create a new one, all of the fields are gone...

I recall this happening to me and it was scary.

I suggest making a copy of the form.

Then I suggest disabling your form rules (not delete!, Just disable) and see if the fields come back.  If they do, then gradually turn them back on until you isolate the cause.

When it happened to me I think it had to do with adding a conditional drop down field to a form which was misbehaving.  Are you using conditional drop downs?

Do you have conditional drop down fields that you just added to the form?

I did but I went ahead and deleted all of the rules, but that did not bring anything back either.

I can see all of the fields that I created but when I click on create new client the form is no longer there.

I was asking if you are using Conditional drop down fields.  ie where the drop down in a field depends on the selection of a previous choice on the from?

yes. But I figured it out... I Did It!! Oh No!!! I hit clear and then when it asked if I wanted to clear form elements or dynamic rules I clicked on both!!!!! :( I guess I am starting over from scratch!!! Oye!!!!

The only way would be to ask QuickBase to restore your app to yesterday.  All data entry would be lost since last night.

Look at the top of the page for HELP and Manage Support Cases and put in a ticket.
