Whenever I try to open an exact form from Quickbase, after I put in my password, I get an error message saying "type mismatch" and it won't let me access Quickbase to open the form.

I have successfully opened exact forms from Quickbase several times in the past, so I do not understand what caused this. I am certain my log in info is correct.
I have even deleted the original template and downloaded a new one, a few times, but still have the same issue.
I need to make changes to an exact form, so help on this will be greatly appreciated!

What version of Exact Form are you using?  Try downloading: https://www.quickbase.com/tools/QuickBase%20Exact%20Forms.dot

Does the issue still occur?

You could always try editing the exact form code directly (I would advise you copy the page first incase you make a mistake), theyre stored as code pages (where dashboards are saved) under the main section of the app, I find this is often easier than trying to play around with the exact form once it's been setup as its very temperamental at times trying to connect to QB and download the stored exact form using the template. I use CTRL+F to search for the sections I want to change for quick navigating.
