Check box that duplicates fields within a table

I want to add a check box, that when checked, it duplicates certain fields under the same section heading. And I want to be able to do this multiple times. I attached an example of what I'm trying to accomplish.

    You would make a form rule that says

    When Multiple conditions are true

    when all of

    Add Claim#2 is checked

    Claim 2 is blank

    Claim 2 FDOS is Blank 

    Claim 2 TDOS is blank

    ACTION

    Change 

    Claim 2 to the value in the field in Claim 1

    Claim 2 FDOS to the Value in the field Claim 1 FDOS 

    Claim 2 TDOS to the value in Claim  1 TDOS
