I'm also having an issue with editing an exact form I've created. When I attempt to sign in it asks for a QuickBase Application Token, which according to the post above would appear it shouldn't even need. If I enter the app token (which isn't required by the app) or leave that field blank, I get an API Authenticate timed out error. Thoughts?

Correct, it is documented that exact Forums doesn't play well with app tokens.
If you must have application tokens, then you need to switch to something like Juiced EF+.

Turns out I was using an out of date template.
Have since got the updated template.
But now I find the form only accesses and displays the field values if I turn the app tokens off.....
