Are you wanting to have a link to the form from the Home Page?

------------------------------Thanks for responding. I wanted to have one home page for our sales reps. I needed to make it really simple so I made a for that has only the information they would need on the road. The form is perfect - I just want that to be their home page. Thanks Lori

------------------------------It's an unusual request, but you can make a Home Page and drag in the Web Page widget.
Then configure the widget with a copy paste of the URL to the record that you want to display, but add the suffix to it of
&ifv=1
If the page does not render, then you will need to put in a support ticket to ask Quickbase support to "Enable iframes for your Realm"
This all assumes that your goal is to embed a single particular record on a Home Page.
