Checkbox in Docusign

I have the Docusign integration working well except 3 issues.  Can anyone help?

1. The checkboxes in the Docusign Template are not showing up to map.
2. My dates are showing up with long time stamps.  Can I make it just a date?  The proper field type and formatting is selected in Quikbase.
3. My currency is showing up with no comma and one decimal.  The proper field type and formatting is selected in Quikbase.
