Help with Form Wizard

I need to create a form to put on my website which new clients can enter their data and it will put the data into Quickbase.

I followed the couple Q&A links i found here but it still comes up blank when i try to create the html. I have created a Role for EOTI and it still isnt working. 

Any help would be greatly appreciated, Also if there are any freelance developers that would like to work part time helping build out some features i need it would save me a lot of time, and probably a few stressful moments.

Id appreciate any help!
