Change Notification with Old and New Values

I'm in search of a way to track and notify users when a the data in a field changes. As an example, I'd like to send an automated notification when the name of a project is changed...

the syntax for referring to the old value in a field is this
Project Name changed from [old.Project Name] to [Project Name]
just prefix the field name with old. Inside the [Square Brackets]
