Customer Data with buttons for Auto Fill/Exporting to word template or other

Looking for an app that will allow input of customer data, with status of customer request, track missing items, but then hopefully have a radio button to auto populate either a word doc or other prog...

New to quickbase and testing it out. I've been viewing some apps in the app exchange and also working by creating my own.  The Qu-Soft Contractor one has some items in there but unsure how to replicate some of the items.
