Duplicate lookup fields on 1 form

Question

I am trying to create multiple lookup fields within 1 form, using 1 relationship. Is this possible? here are the screen shot, so basically just want to create another "Employee Name" , "Employee ID" and "Title" lookup field.

You will need to create a new relationships similar to the first one.
