Exact Form Print Based on Date Field

Is there a way to have a print button to print a certain exact form based on a date field?  I have a form that has a print button on it to print the exact form.  Often, field names on the form will need to be changed based on Medicare guidelines.  So effective March 1 I need to make changes to a few of the field names.  Rather than creating a new exact form with the changes and a form rule that says if the review date is prior to March 1, show print button A otherwise print button B, I'd like to be able to have one print button that can see the review date and know which exact form to print.

I hope this makes sense.  Is this possible?
