Exact forms displaying blank page; Nothing changed...

I have had and exact form running just fine for 2 years and now, just recently, it is not working.

When I click the link for the print function, it just shows me a blank page.
The page it is referencing is still there and I haven't monkeyed with anything related to the form. 

Help?

    This was solved. 

    I had the App token requirement turned on.

    I'll bet I can guess the problem ....

    You have what is now Rich-Text fields. And when those are pushed into Rich-Text Formulas, the formula ignores them/breaks.

    Fix: All you have to do is open the Rich-text formula and re-save.

    $5 on the table.

    I am having the same issue, however my app token requirement is off and I created the Exact Form with the Rich Text Formula fields. 

    When I put a button to the exact form page, without an associated Record ID, on a dashboard page, the blank template will pull up. However, the URL button I have on my form that is supposed to open the exact form with all fields populated from the associated record suddenly stopped working. 

    Any ideas?
