Form Header on Each Printed Page

Hello, 

I have a simple form in Quick Base that contains a header including my company's logo and contact information.

When I print or save the form as a PDF, the header only displays on the first page of the form.

Is there a way to link my form header to each page if I were to print or save the form?

The header is a text element with HTML limited properties.
