Are the checklists being conducted in a form view or grid edit view?
If its on a form view, you could use form rules to push notes, then have a formula fieldwhich counts the # of line breaks in that text field to calculate the # of issues.
If its on grid edit, you can do the same thing, however you would need to create a formula text field which would detect the checkboxes then push the correct note to the notes field.

The form is completed in form view. I should clarify, I only want to count the fields that have an entry in the notes field, not the total number of entries in the notes fields.

------------------------------- If you are just counting the # of check boxes which are filled, you can create a formula numeric with a long If statement to count.
ie
var num Count = If([checkbox1]=true,1,0)+
If([checkbox2]=true,1,0)+
If([checkbox3]=true,1,0)+
etc

------------------------------- There are no checkboxes on the list, each item on the check list has a dropdown with values of pass, fail, N/A and a notes field that is required if the item failed. The current summary report is counting the records where the notes field is not empty. I am not looking for the total failed in each record, rather a summary of which checklist items failed during the month so we can identify key failure points. Also, trying to count the checklist items with a value of fail will not work. The checklist is updated after each failed item is corrected. This part of the app is still fairly young and I may add a checkbox and form rule to add a check if an item fails and make sure the rule only fires when the change is from false to true.
