How do I add a map to a form

I created a map report to display utility pole locations for a work order. I would like to have the map show as a element on my form. If in-bedding will not work then  add as a button that would open a pop up window of the map. 

I tried to add a report link field but this only returned the table grid report

Any thoughts?
