How do i only show two fields per row ?

In the forms setting, i am only able to select "same row" to all my fields which groups all my fields in the same row.  I want to be able to display two fields per row.  My goal is to have 5 rows of checkbox and text box beside each other.

example: 

checkbox1     text1
checkbox2     text 2
checkbox3     text 3

    You need to select same row only on the text fields if it is to match the scenario you show.  The same row check box is for that field to be on the same row as the previous field.  Leave a check box unchecked if you want the field to start a new row.
