Pipeline creates a blank record

Testing Pipeline for the first time and when we used A-Record Created to B-Create Record, the record was created but only after we hit the Sync button. Looking further into this, we realized we needed to do A-Record Created to B-Query to C-Create Record. Now the record is created automatically in the new table but all the fields are blank. We are unable to figure out why. In Step B Query we do have one condition added that a particular multiple choice field must be 'X'. Our A-Record Created does contain this value so we don't think this is the issue. Does anyone know what else we need to look at to figure out why a blank record is being created?
