Please help me to retrieve a form that was deleted from Quickbase

Hi Oksana,

In order to restore lost data that was deleted from a Quick Base application you would want to reach out to one of the Quick Base account admins at your company. They would need to submit a case to our Care team to ask for the application to be restored back to before data was deleted from the app. Since that is such a serious change those requests need to come from the account admin. 

When they are ready to submit a case to the Care team here at Quick Base they can do so when signed into Quick Base by clicking the "?" question mark icon that appears in the upper right hand corner and selecting manage support case or directly here. Thank you very much for your time today Oksana.
