"Progress" Bar for a Form

I'm looking to create a Progress Bar for a survey that my users are filling out via a QB form. Has anyone does this before that can offer any insight or best practices?

My current plan is to just take the question the user is on divided by the # of questions, and then show/hide a numerical % that changes for each subsequent question. Ideally, would love to have some sort of graphical representation to show this.​
