Quickbase form on site causing a redirect

Hi,

I have an issue that I was hoping someone with better HTML knowledge might be able to help me with. 
I have a form on my site that feeds into our QuickBase app. In terms of feeding back the info it is working but for some reason there seems to be a redirect embedded in the HTML meaning that our 'thank you' page is giving a 404 error.

I would be very grateful for any help anyone could provide.
