You will likely need to redo your development work now on the version of the app which has the current data, ie the old app.

I suggest that you make a copy of your app and then experiment with using the sandbox to learn about it.
But, the idea of the sandbox is to be able to make change in a controlled Sandbox environment and then Promote them Live when you are ready, preserving all the data in the production copy of the app.

Is this still true in 2023? Is there no way to copy my new dev work from the test app to the live app (excluding the data of course)

If you enable the Sandbox feature nd do the dev work in the sandbox, then it can be promoted to live. I have not worked with the sandbox much. At one time, when enabling the Sandbox, Saved Table to Table copying were disabled. Not sure if any other caveats. Maybe read up about the sandbox feature annd ask support about any known caveats.
But for the development work, you've already done, if it is just in a copy of the App, not using the sandbox features, and there is no magic way to apply your development changes into the live app.
