Trying to show the field that changed in a notification

I have set up notifications (single record) to send when one of the field data changes. How do I show which field changed? Also, [Last modified by] is not showing in the email. This is my message: ...

And, your [Last Modified By] should work. Wonder why? Check the field properties to make sure there are no restrictions.

If you include a copy of the form the field(s) which changed will be highlighted in red,.

  Thx Mark. The form has alot of data in it. I guess I could create a new form with only this info, tho.

  I have created a new form to be used for this notification. It is not the default form for the table, however. So, when the notification is received...and I click "here", the main form opens...not the new form?
