Quickbase Newbie - How to display multiple fields from a lookup table

Good day

I want to capture the code (non key field) in a table but want to display the code description on a form/report. 

Any assistance would be appreciated.

    No problem.

    Make a relationship where one "Code" has Many records. Then add lookup fields to the relationship.
