How do I add a map to a form

I created a map report to display utility pole locations for a work order. I would like to have the map show as a element on my form. If in-bedding will not work then  add as a button that would open a pop up window of the map. 

I tried to add a report link field but this only returned the table grid report

Any thoughts?

    Just make an unfiltered map report and set the form properties to have the report link field show the records in the form and to use the unfiltered map report.

    I have everything setup but I don't see were to set the link to the unfiltered report

    You set that on the Form where you have that Report Link field.
