New App, need to get info from another app

I am in the process of essentially starting over with our project management database and need a little help. A consultant basically built the two applications that we use, Grants (which is all the financials) and Projects & Activities (everything else like contracts, project info).
We have started a brand new application and have gone all the way to Activities, which is the last step before Contracts and Grants. I need help getting information from our existing app to our new one. Because we will be re-creating the Grants application as well, I don't know how successful it will be since each app talks to the others. Help!
