Can someone help point me in the direction of doing the project condition field? Is this an image or a formula? Looking to set this up to add the health of a project into a report and the only instructions I could find was around color coding the field. Thank you!

Depending on your what project condition is controlled by, such as a status field, you just use an If statement to control something like this:
