Most likely, it's a problem with the text being inserted and how Excel is interpreting the CSV data - namely the column delimiters. I'm guessing your weird column break issue occurs when the user has entered a comma into the text field. Unfortunately, I don't know of a reliable way to fix that except to strip any commas from the text using a formula and then downloading from a report that uses the formula instead of the original text.
