Automatically Create New "Client Health" Records Each Month

Here is a screenshot, as you can see, Programs and Team are both connected with a 1:Many to the Client Assignments table which is our join table. This is where we are able to give programs multiple team members and give team members multiple program assignments. I currently have Client Health connected to the Programs table, but I believe I should connect it to the Client Assignments table instead.

Edited to add: The type of record I want to create is a new client health record for each client assignment that exists. Essentially I want an automation to create new records across each client assignment where all the fields are blank except for "Program Name," that way client teams can easily click into this report at the end of the month, fill out their client health fields via grid edit and save. Essentially, i'm trying to make their end of client health as easy as possible, we just want them to fill in the blanks and hit save.
