Qrew LegendLet's start with the basis. Are these manual checkboxes or formula fields? When you look at the record are they checked. Like maybe they are not showing as checked because they are not checked.
They are manual checkboxes. The only difference I'm able to find between the checkboxes that work correctly and the ones that don't is that Task Table In Investigations isn't listed in the usage list.

Perhaps there is some confusion on field names and you have multiple check boxes with similar names and some of the check boxes are on the form, but similarly named fields are on the report.
Here is the list of the fields that are check boxes. I'm wondering if it would work to duplicate a check box field that displays correctly, remove the one that doesn't, and just rename the duplicated field to the one I removed?
