The footer information can be removed by going into form properties and deselecting
Show built-in fields
as for the header information identifying the record there is a setting in the Advanced Settings for the table for the Record Identifier.

| The record title displays at the top of all forms for this table to identify the record. Select a field below to change the record title. |
Could create a dummy formula text field called [blank] no formula and use that as the record identifier.
I would also mention that if you were getting header or footer URLs or timestamps being printed by your browser than that is a browser setting to suppress those headers and footers when printing.
