    Hello, is there a way to remove the app name, table name, and record number above the table as well as the information of who created/last edited the records?

    The footer information can be removed by going into form properties and deselecting
    Show built-in fields

    as for the header information identifying the record there is a setting in the Advanced Settings for the table for the Record Identifier.

    Record Title
    The record title displays at the top of all forms for this table to identify the record. Select a field below to change the record title.


    Could create a dummy formula text field called [blank] no formula and use that as the record identifier.  

    I would also mention that if you were getting header or footer URLs or timestamps being printed by your browser than that is a browser setting to suppress those headers and footers when printing.

    Thank you so much for your reply and apologies for taking so long to respond. Everything you mentioned works perfectly! Is there a way to hide the app name and table name that appears at the top of the table?

    I don't see a way to control the form in the email when %recdata% is used. 
    My only other thought would be to not include copy of the form but rather provide a link to display the record but include a suffix in the URL of &ifv=1 to suppress the Quickbase headers.
