Anybody know of a decent facility management app on the exchange that could be used and modified to track preventative maintenance schedules and tasks.

I have an app which is not in the exchange but that I can share with you.  Contact me via the information in my profile.  The app basically tracks a Preventative Maintenance schedule for each equipment piece, and users log Maintenance entries when the PM is done, thus resetting the reminder date.
