I am not sure what the last picture being empty, has to do with the checkbox? Can you explain the relationship? Why does the blank information have any affect on the checkbox from picture 1?
Also, if you go to the field properties of the checked box, can see if it is a formula checkbox, and if so, post the formula?
If it is not a formula checkbox, and you go to the 'usage' tab and post what is there?

It must be a formula checkbox. I played around with a sample app and if the formula checkbox is blank there is no checkbox. If I put a formula in and it is satisfied there is a check in the checkbox. The other type of checkbox is just a simple checkbox, but from the properties window, I see that it's value can come from a lookup field and it can't be changed!

Editing rights is different than having admin rights and being able to actual view the field properties. Like Bob mentioned, it is quite likely that you are looking at a formula checkbox, however you will need to speak with your QB developer to know for sure.
